HistoryA girl complian with hair loss about 1 month ago.she deny febrile arthralgia or sickness before.no underlying disease.and deny disorder in her family.
Physical examination
A girl looked anxiues .good mental screening.
Not pale, no jaundice,no Beau’s line or pitted nail.thyroid not enlarge
No lymphadenopathy,liver and spleen impalpable.
No vitiligo
ผู้ป่วยรายนี้มาด้วย non scaring alopecia ลักษณะผมร่วงเป็นย่อม ๆ เข้าได้กับ Alopecia areata มากที่สุด อื่นที่ต้องซักประวัติคือ fungal infection bacterial infection chemical ผลการกระทำของตนเอง เช่น Trichotillomania ความผิดปกติของต่อมไทรอยด์ systemic disease
Approach alopecia
สาเหตุยังไม่ทราบแน่ชัด เชื่อว่าเกิดจาก autoimmune พบว่าประมาณ 1 % ของผู้ป่วยที่มาพบแพทย์ด้วยโรคผิวหนังเป็นโรคนี้ พบในผู้ป่วยอายุ 20 -30 ปี ในระยะ active บริเวณรอบ ๆ จะดึงหลุด ส่วนบริเวณอื่นดึงไม่หลุด ตรวจเล็บอาจพบ pitted nail หรือ dystrophic nail
การรักษา
ส่วนใหญ่หายเองภายใน 1 ปี แต่ประมาณ 30 – 40 % ของผู้ป่วยเกิดโรคซ้ำ
การรักษาอาจใช้ potent steroid หรือ intralesional steroid ไม่เกิน 20 mgต่อครั้ง ทุก 4-6 wks นอกจากนี้อาจรักษาโดย PUVA ทา minoxidil
